Actually, on second thought, I think we should open up a new ticket.  The
reason I said this is that, just now, when I talked with Bank of the West
about my missing 1099 form, they said it would take 2 weeks for me to
receive it.  I don't want to wait too long because otherwise we'll have to
file an extension for our taxes.
