RE:HR-275830-F2H1R7 FW: Macrosoft Insurance Coversion for an Ex employee

I submitted it almost one month ago and they never received the Employer
Section of the paperwork from Macrosoft.

I called them today to ask for a deadline extension.  I'm hoping that the
deadline will be granted.
