After getting my statement from US Bank, I discovered that they charged
$2.50 per transaction from the ATM machine at Walgreens.
So, with the $5 charge for the ATM card replacement, this cost me
$2.50 + $2.50 + $5.00 = $10.00 . Grrrr...
