Virtualbox and XTEST fake input

Hi.  I saw http://forums.virtualbox.org/viewtopic.php?f=3&t=32383 .  I've
been trying to get x11vnc working but mouse buttons (other than 1)
are not working correctly.  In particular, the right mouse button fails
and sometimes crashes x11vnc.

I noticed in http://www.virtualbox.org/ticket/7152 you mentioned "XTest
works just fine with mouse integration off".  How does one do that in
Virtualbox?
