I have no idea what sort of infrastructure postfix requires, but the
off-the-top-of-my-head-guess says that it is looking in the wrong file for
your password.
Also, you should make sure you're using a supported hash method.  (When I
set up CRAM-MD5 with my dovecot, that was one problem I encountered.)
