I had an ankle sprain about 4 years ago and sometimes my ankle does act up. What I do is put the clicker at a right angle to my foot and put a piece of 2x4 in front of the clicker. Then I rest the heel of my foot on the 2x4 and press the left or right front part of my foot to press the clicks instead of the usual method of placing my foot sitting on top of the clicker pedal. This seems to allow my ankle to relax and doesn't cause pain.
